Testing Group Petition for Research Degree Graduate Students

Please note this form is only for students in the research degrees MFA, MA, MS, PhD, and DMA. If you are in another research degree such as MEN, MPS, MMH, or a non-degree program you are not eligible to use this form. If you are in an ineligible program and have questions regarding your testing frequency, please submit a ticket to the COVID Support Center.
  • All graduate students in research degrees (PhD, DAM, JSD, MS, MA, MFA) in the Ithaca area are required to participate in Cornell’s coronavirus surveillance testing program.  Graduate students will be assigned a surveillance testing frequency of one test per week.
  • Research degree students who are on campus two times per week or less, with limited interactions with other people, and no in-person course enrollment may petition to be placed on a reduced testing frequency of one test every other week (bi-weekly testing).  Allowable activities would include individual on-campus research or scholarship activities with no in-person group or collaborative work.
  • Research degree students who are functioning 100% remotely for their research, scholarship, and coursework, and for whom coming to campus for periodic surveillance testing would be a hardship (e.g., because of at-home childcare responsibilities, personal health considerations, risks to vulnerable members of household or community, or transportation challenges), the testing requirement may be waived so that they are not required to participate in surveillance testing.
